Helping you access the seniors support services you need in Mississauga
Mississauga Health (Mississauga Ontario Health Team) is pleased to bring together passionate health care organizations from across the system to best support older adults in our community who are 65-years-old and up.
Through the Seniors Navigation Portal, older adults are connected to services through a centralized, ‘one-stop-shop’ digital system. With the older adult population growing and increasing in complexity of needs, Mississauga Health recognized the need to develop a digital tool that could help navigate this population to services seamlessly by connecting clients to intake and booking appointments.
The goal of the Mississauga Health Seniors Navigation Portal is to help older adults easily connect with the services they need or access the support of a care navigator who can assist them. Some of the services provided include adult day programs, daily living supports, caregiver assistance, and much more.
While the first phase of this Navigation Portal is focused on older adults, we hope to expand this service to even more members of the community. Mississauga Health is focused on re-designing care with the person at the centre by focusing on our priority populations: Older Adults, Palliative Care, and Mental Health and Addictions.
Together, we will improve the health and wellness of all people in our communities by increasing access and connecting care across the system.

Our navigation services are free for anyone living in the Mississauga region. There may be a fee for the services you are directed to, as this is determined by each organization providing services.
To book an appointment with a Care Navigator, start by filling out this 5-minute questionnaire.
After you complete the questionnaire, you may be invited to book a phone appointment with a Care Navigator. The Care Navigator will call you at your chosen appointment time to help determine what services will be best for you. The Care Navigator will then send a referral on your behalf to the services you need, and the organizations providing these services will be in touch with you directly.
