For Service Providers
Clinicians and service providers can access services for their senior patients from a collection of more than 20 trusted health care agencies. These agencies are working in partnership with the Mississauga Ontario Health Team.
How does it work?
Sending a referral is easy
Fill out the form
Fill out your client's information to connect them with a service navigator.
Navigator contacts client
Be confident that referred patients will be contacted by a Senior Care Navigator. The Navigator will call the client within 2 business days.
Clients are connected with service
Clients will be booked into relevant services from a network of trusted agencies. The option to launch a coordinated care plan across agencies will be provided to the client.
