    Our navigation services are free for anyone living in the Mississauga region. There may be a fee for the services you are directed to, as this is determined by each organization providing services. 

  • How do I connect with a Care Navigator?

    To book an appointment with a Care Navigator, start by filling our this 5-minute questionnaire.

  • What happens after I complete the questionnaire?

    After you complete the questionnaire, you may be invited to book a phone appointment with a Care Navigator. The Care Navigator will call you at your chosen appointment time to help determine what services will be best for you. The Care Navigator will then send a referral on your behalf to the services you need, and the organizations providing these services will be in touch with you directly.
