The Mississauga Health Seniors Navigation Portal helps older adults continue to live well at home by connecting you to health care providers in Mississauga, before your health becomes an emergency.
Whether you are concerned about your health, or the health of a loved one, our team is here to support you.
After reaching out to us, you will be paired with an expert Care Navigator who will help connect you to the services you need and support you through the process.

Depending on your results, you might be directed to book an appointment with a Care Navigator, or directly with a community support provider. You will receive an email confirmation once your appointment is booked.
Care Navigator appointments:
Clients with complex needs will meet with a Care Navigator to discuss your questionnaire results and ask additional questions.
Community Provider appointments:
An intake staff member will share more information about the program you've been matched with to confirm it's a good fit.
No need to repeat your story; the provider will have a copy of the information you shared through the questionnaire.
Set up with an online care plan on
the Careteam platform, where you
can see what needs to be done, find relevant resources, and collaborate with your family members and health care providers.
Connected to
one or more community support services that can help provide wraparound care for you or your loved one. Additional intake appointments may be coordinated at this time.
Stay nourished with services such as meal delivery and group dining.
Stay safe and healthy at home with housekeeping and home maintenance.
Social interaction and safety checks for those who are socially isolated.
Receive comfortable care at home from trained personal support workers.
Support in a group setting for caregivers, stroke survivors, grief support and more.

Our navigation services are free for anyone living in the Mississauga region. There may be a fee for the services you are directed to, as this is determined by each organization providing services.
To book an appointment with a Care Navigator, start by filling out this 5-minute questionnaire.
After you complete the questionnaire, you may be invited to book a phone appointment with a Care Navigator. The Care Navigator will call you at your chosen appointment time to help determine what services will be best for you. The Care Navigator will then send a referral on your behalf to the services you need, and the organizations providing these services will be in touch with you directly.
Our online tools make it easy for you and your service providers to see all the care you are receiving, in one place. This makes ongoing communication across the care team simple and enables smoother care transitions and transparency across both health care professionals and family members.
