The Mississauga Health Seniors Navigation Portal helps older adults continue to live well at home by connecting you to health care providers in Mississauga, before your health becomes an emergency.
Whether you are concerned about your health, or the health of a loved one, our team is here to support you.
After reaching out to us, you will be paired with an expert Care Navigator who will help connect you to the services you need and support you through the process.
Who is eligible?
Older adults 65+
older adults who have had a recent change in health condition or require additional supports
or their caregivers
caregiver burnout is common; we support all professional and family caregivers
living in Mississauga
the person requiring care must live within Mississauga / Malton city limits
who need help at home
we can connect you to services like meal deliveries, respite, or transportation to a medical appointment

Connecting you to help
Complete a 5-minute questionnaire
Click the 'Get Started' button below to answer a few questions. This will take less than 5 minutes, and will help us understand your needs, or the needs of your loved one.
Depending on your results, you might be directed to book an appointment with a Care Navigator, or directly with a community support provider. You will receive an email confirmation once your appointment is booked.
Attend your phone appointment
Care Navigator appointments:
Clients with complex needs will meet with a Care Navigator to discuss your questionnaire results and ask additional questions.
Community Provider appointments:
An intake staff member will share more information about the program you've been matched with to confirm it's a good fit.
No need to repeat your story; the provider will have a copy of the information you shared through the questionnaire.
After your first call,
you may be:
Set up with an online care plan on
the Careteam platform, where you
can see what needs to be done, find relevant resources, and collaborate with your family members and health care providers.
Connected to
one or more community support services that can help provide wraparound care for you or your loved one. Additional intake appointments may be coordinated at this time.
